Some issues that may be found to be causing unsafe attitudes and behaviors include lack of management and front line supervisor commitment, unclear procedures, inadequate training, insufficient or inadequate safety equipment and supplies, and lack of employee involvement, enforcement, disciplinary action, and incentives.
